Water/Wastewater/Sanitation Service Forms

  • Residential Contract for Service - Required application for Residential Utility Service. An application must be completed and delivered to the Utility Billing Customer Service Department at City Hall in person or by email (please see FAQ - "Do you accept applications to start water/wastewater/sanitation service by email" for instructions for the email option.) We are located at 16 West 9th Street in Shawnee, Oklahoma. If you choose to come into City Hall, you can print and complete an application and bring it with you.  You also need to bring with you a copy of your deed (if you own the home) or a copy of the lease (if you rent/lease the home), valid photo identification, and deposit amount.
  • Business/Commercial Contract for Service - Required application for Business/Commercial Utility Service. An application must be completed and delivered to the Utility Billing Customer Service Department at City Hall in person or by email (please see FAQ - "Do you accept applications to start water/wastewater/sanitation service by email" for instructions for the email option.) We are located at 16 West 9th Street in Shawnee, Oklahoma. If you choose to come into City Hall, you can print and complete an application and bring it with you.  You also need to bring with you a copy of the deed (if you own the property) or a copy of the lease (if you rent/lease the property), valid photo identification, and deposit amount.
  • Application to Add Co-Occupant to Existing Account - Allows you to add a co-occupant to an existing account.
  • Service Waiver - Allows the original applicant to transfer the account to a new applicant and the new applicant assumes all responsibility to the existing account . This Waiver must be completed in person to the Utility Billing Customer Service window at City Hall and must accompany a Residential Contract for Service for new applicant.
  • Authorization for Automatic Bill Payment - This will allow the City to automatically draft your water bill payment from your designated account on the due date, please provide a voided blank check. You will continue to receive a copy of the bill each month, based on your chosen delivery method. 
  • Landlord - 30 Day Contract for Service - Required for 30-day water/wastewater/sanitation service not requiring a deposit (only available to landlords and only for 30 days; any time after 30 days, a deposit is required).
  • Landlord - Auto Reinstate Application for Service - This application allows water/wastewater/ sanitation service to automatically revert back to the Landlord when a tenet leaves the service address (a deposit it required).
