Case Management
Our case managers work closely with you to help address the issues keeping you from achieving your goals. Case Managers are here to help you get connected with the resources that are available. All records containing the personally identifying information of any individual or family in this program will be kept secure and confidential. If you need case management services, you can call (405) 878-1938 and speak to the Case Management Specialist.
